Please can you tell me how I play the D & C , and why is the C not directly under the D , like the following double notes D and B ? Thank you.

Press D and C at the same time, keep holding the D, release C and press B. What you have there is something that starts with D and C and ends with the same D (held throughout) and B.

The D and C are offset there so you can see that they are different notes. If they were right on top of each other you wouldn't be able to tell that the D is a half note because it would be partially overwritten by the C under it.

In addition, the offset shows that there's an emphasis on the D when you strike it. Hit D harder than C, in other words.

You play the D and C together. The C would halfway obscure the D if you wrote them vertically. You can't suddenly put the C leger line lower than its usual position as it would upset the consistent system of lines and spaces on the staff.
Imagine the D and C an octave higher within the main staff. They have to be written offset so you can clearly see the note values. You can't have the lines on the staff doing sudden dips or zigzags in order to get all notes of a chord lined up!
